Create CP/M images and disks using MMC64

• initialize .d64 image: cformat ~/ladder.d64
• write CP/M files to image: ctools ~/ladder.d64 p LADDER.*
• copy ladder.d64 to FAT16 formatted SD-card and insert into MMC64
• browse to the new ladder.d64, insert DD disk into Commodore drive and press enter
• wait while ladder.d64 is copied to the floppy disk
